MEDICAL COVERAGE
Three (3) medical plans under Anthem Blue Cross. You pay your share of the premium through pre-tax payroll deductions. Company provides employees with an allowance to use on medical benefits to cover Employee Only portion of medical costs. You pay 100% dependent coverage.
DENTAL COVERAGE
One plan offered through MetLife DPPO Plan. You pay your share of the premium through pre-tax payroll deductions. Company pays 100% for dental coverage for Employee Only portion of dental costs. You pay 100% dependent coverage.
VISION COVERAGE
One plan through EyeMed. You pay your share of the premium through pre-tax payroll deductions. Company pays 100% for vision coverage for Employee Only portion of vision costs. You pay 100% dependent coverage.
WORKSITE VOLUNTARY PLANS
Voluntary Accident, Critical Illness and Hospital Insurance plans are provided by MetLife. These plans help with your out-of-pocket expenses. Most of the plans allow you to elect coverage for yourself and your family. Some of the plans have a wellness benefit. You pay the premium through payroll deductions.
